Our composition of care is provided in accordance with APIE [Assess, Plan, Implement and Evaluate] nursing model in order to meet the requirements of our service. APIE is a cyclical process. The APIE model encompasses the service users’ needs, choices, aspirations and goals, and monitors progress and development. The process evolves overtime to reflect their achievements, development, changes in needs, choices and aspirations, of each service user.
