SERVICE MODEL

8

Our composition of care is provided in accordance with APIE [Assess, Plan, Implement and Evaluate] nursing model in order to meet the requirements of our service. APIE is a cyclical process. The APIE model encompasses the service users’ needs, choices, aspirations and goals, and monitors progress and development. The process evolves overtime to reflect their achievements, development, changes in needs, choices and aspirations, of each service user.

The core values of personal privacy, dignity, individual rights, independence and empowerment are at the heart of the service we provide.

Service users make their own choices in the following areas of our service provision:

  1. Daily living skills – including the right to privacy, ‘own’ space, personal hygiene, respect for possessions and encouragement of choice for their room and home

  2. Faith – service users who are religious will be supported to practice their faith at home and in places of worship

  3. Home running – meal choice and preparation, involvement in household chores, shopping and gardening

  4. Activities – dependent on service users’ needs, a full programme of activities will be made available, for hobbies, interests such as baking and gardening, sports and access to education and employment where appropriate

  5. Outdoor pursuits – dependent upon need, ability and preference, opportunities are available for all service users and will be provided by qualified instructors as necessary. Examples: Hiking, Fishing, Climbing, Cycling & Riding
